At-Risk Transition Coordinator

REQUIRED QUALIFICATIONS:
- Bachelor’s Degree in Education or related field preferred
- Knowledge of residential and institutional facilities
- General working knowledge of criminal justice system
ADDITIONAL PREFERRED QUALIFICATIONS:
- Formal training or degree in Sociology, Psychology, Criminal Justice, Education or other closely related field
POSITION SUMMARY
To assist the Special Services Director in tracking and coordinating educational transition services for students who enter and exit the district schools from placement and provide support to various at-risk programs.
PRIMARY RESPONSIBILITIES
- Coordinate transition services for all court/DFS placed District special education students who enter and exit the district from residential and institutional facilities
- Coordinate educational services for students on IEPs who are placed in residential facilities, DRC, SUDS, and/or Acute Placement.
- Coordinate services and scheduling of transition meetings between personnel of exiting
facilities and entering schools
- Coordinate and assign tutors and all supporting services for students placed on Homebound status for K-12 and expelled students.
- Attend and participate in monthly meetings/staffing’s of all district students currently placed in out-of-district placements
- Coordinate and chair monthly meetings for students placed on suspended expulsions and coordinate supplemental services as necessary
- District liaison between Day Reporting Center (DRC) staff and district special education students.
- Monitoring and attendance record keeping for all students from throughout county that
attend DRC
- Track, monitor and calculate ADM for all parentally placed, court placed and district placed students for K-12
- PowerSchool SIS tracking in coordination with district SIS Coordinator
- Assist in internal auditing and review of SPED Files
- Attend weekly district meetings and assist in identifying and coordinating services and
interventions for at-risk students
- Serve as a MANDT trainer for the district, or willing to undergo training to become one
- Process state parent surveys after SPED meetings are completed
- Survey graduated special education students annually for Indicator 14 Report
- Collect student counts for the WDE 549 School District October Caseload Count
- Performs such other duties as assigned

EVALUATION
Performance on this job will be evaluated by the Assistant Superintendent or the Student Services Director in accordance with school board policy and administrative regulations on evaluation of certified or classified personnel.
SALARY SCHEDULE
This position will be paid according to the approved Classified Administrative Support Salary Schedule.
TERMS OF EMPLOYMENT
Teacher School Year Calendar, 30 (10 Days before and 20 days after)
