Key Responsibilities
- Assess patients' psychosocial status, strengths, and areas of need that may affect rehabilitation and optimal treatment outcomes.
- Collaborate with patients and the healthcare team to identify effective interventions that help meet rehabilitation and treatment goals.
- Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life.
- Assess patients' knowledge of kidney disease and provide supportive counseling services as needed.
You will also be responsible for providing information and referrals to community resources to facilitate optimal treatment outcomes, maintaining current knowledge of local vocational/educational rehabilitation programs, and assessing patients' awareness of advance directives and facilitating discussion with the healthcare team and family.
