What are the responsibilities and job description for the Master Social Worker position at Fresenius Medical Care?
Purpose and Scope:
Fresenius Kidney Care Social Workers provide psychosocial services to patients undergoing dialysis. We work with the healthcare team to promote positive adjustment, rehabilitation, and quality of life for our patients.
Key Responsibilities:
- Patient Assessment and Care Planning:
- We assess patients' psychosocial status, strengths, and areas of need that may affect rehabilitation and optimal treatment outcomes.
- We participate in care planning with the patient and healthcare team to identify effective interventions to help the patient meet rehabilitation and treatment goals.
- We use Fresenius Kidney Care patient education programs, social work theory, and methods to address barriers and meet patient treatment goals.
- Quality:
- We provide psychosocial support and interventions to address non-adherence, quality outcome, and quality of life concerns for all patients based on acuity level.
- We participate in monthly Quality review meetings with the interdisciplinary team, reporting on quality indicators related to adherence, such as Missed and Shortened Treatments, Quality of Life Trends, and Service Recovery.
- Patient Education:
- We assess patient knowledge of kidney disease for barriers that may affect adherence to treatment, working with the patient, family, and healthcare team to provide education tailored to their needs.
- We facilitate the transplant referral process and collaborate with the interdisciplinary team on transplant waitlist management.
- We provide ongoing education to patients and families regarding psychosocial issues related to End Stage Renal Disease (ESRD) and available support services.
- Patient Admission and Continuity of Care:
- We review patient rights and responsibilities, grievance information, and other facility policies to ensure patients understand their rights and expectations.
- We understand the referral and admission process, supporting the clinic in addressing patient needs for scheduling to maximize adherence and adjustment.
- Insurance and Financial Assistance:
- We collaborate with Insurance Coordinators to address issues related to insurance and payment to dialysis.
- We provide information and education to patients about payment options, including federal, state, commercial insurance, state renal programs, AKF HIPP, and entitlement programs.
- Staff Related:
- We assist with the interview process and decision to hire new personnel if requested by the SW Manager/Senior Manager.
- We work with administrative staff to maintain updated patient resource lists.
Education and Required Credentials:
- Masters in Social Work
- State required license
Experience and Skills:
- 0-2 years related experience
This is an entry-level MSW role, and we offer competitive compensation packages, including a sign-on bonus, annual salary $59,000.00 - $94,000.00, depending on location and qualifications.
