Job Summary:
We are seeking a skilled Social Worker to join our nephrology care team. As a Nephrology Care Advocate, you will provide psychosocial services to patients undergoing dialysis treatment.
Main Responsibilities:
- Assess patients' psychosocial status and identify areas of need that may affect rehabilitation and optimal treatment outcomes.
- Collaborate with patients and the healthcare team to develop effective interventions and improve quality of life.
- Provide supportive counseling services, education, and referrals to community resources as needed.
- Maintain current knowledge of local vocational/educational rehabilitation programs and assist patients in accessing these services.
- Participate in discussions regarding patient DNR status and advance directives.
Required Qualifications:
- Masters in Social Work
- State required license
