Key Responsibilities
As a Psychosocial Rehabilitation Specialist at Fresenius Medical Care, your primary responsibility will be to work with patients, healthcare providers, and insurance coordinators to ensure comprehensive care and support. You will conduct thorough assessments of patient psychosocial status, develop and implement individualized plans, and collaborate with healthcare providers and insurance coordinators.
Your Duties:
- Assess patient psychosocial status to identify strengths, needs, and areas for intervention.
- Develop and implement individualized plans to address patient needs and promote positive adjustment and rehabilitation.
- Collaborate with healthcare providers and insurance coordinators to ensure comprehensive care and support.
- Provide supportive counseling services to patients and referrals to community resources as needed.
Requirements:
- Masters in Social Work (MSW) degree.
- State required license to practice social work.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
