Job Summary
We are seeking a highly skilled and compassionate Psychosocial Rehabilitation Manager to join our team. As a key member of our healthcare team, you will provide psychosocial services to our patients, working closely with healthcare professionals to promote positive adjustment, rehabilitation, and improved quality of life.
Key Responsibilities
- Collaborate with patients and healthcare teams to identify effective interventions that will help patients meet rehabilitation, treatment goals, and improve quality of life.
- Utilize patient education programs, established social work theory and methods, social work-focused interventions, and quality-of-life measurement instruments as part of assessment and care planning to address barriers and meet patient treatment goals.
- Provide monitoring and interventions for patients to adjust to dialysis and achieve optimal psychosocial status and quality of life.
Requirements
- Masters in Social Work degree.
