- Assess patients' knowledge and provide education on the relationship between diet and kidney disease, impacting treatment processes.
- Provide education to patients, families, and caregivers on nutritional status, adequacy measures, and lab results related to their disease process.
- Complete nutritional assessments and work with the interdisciplinary team to develop comprehensive care plans for patients.
- Maintain accurate medical records, including progress notes.
- Offer ongoing assessment and counseling to help patients achieve effective nutritional status.
**Requirements:**
- Registered Dietitian certification from the Commission on Dietetic Registration.
- Board Certified Specialist in Renal Nutrition preferred.
- Current state licensure if applicable.
- 1 year experience in clinical nutrition as a Registered Dietitian; previous renal experience preferred.
