Job Description
The Dialysis Patient Care Coordinator will work closely with the healthcare team to provide psychosocial services to patients treated by Fresenius Medical Care. This includes assessing patient needs, developing effective interventions to improve patient outcomes, and working collaboratively with other healthcare professionals to ensure comprehensive care.
Responsibilities will include:
- Conducting initial and ongoing assessments of patient psychosocial status, including strengths and areas of need
- Developing individualized plans to address patient needs and improve patient outcomes
- Providing ongoing education and support to patients and families
- Collaborating with the healthcare team to ensure comprehensive care and achieve optimal patient outcomes
This role requires a strong understanding of social work principles and practices, as well as excellent communication and interpersonal skills. A master's degree in social work (MSW) and temporary state licensure or provisional LCSW licensure are required for this position. 1-2 years' experience in social work, preferably in a healthcare setting, is preferred.
