As a Patient Advocate, you will work closely with the healthcare team to assess patients' physical and emotional needs, develop personalized care plans, and provide education and support to patients and their families.
You will also facilitate communication between patients, family members, and healthcare providers to ensure seamless care coordination and address any concerns or issues that may arise.
Key responsibilities include:
