HRDI is seeking a Case Manager Assistant to assist in the provision of direct casework pre-assessment for Community Health Services. This role will also maintain contacts with the primary care providers to assure that the clients and his/her family receives appropriate health care services.

3 or more years of experience in the field of behavioral science, social science, or health-related field

JOB DUTIES:
- Responsible for providing direct case management screening/follow-up services to clients at any given time during the fiscal year.
- Maintains ongoing contact with assigned clients to meet the programs standards.
- Responsible for conducting client screening, pre-assessment and assigning caseload
- Conduct Outreach for programs and general office work.
- Ensures that services are implemented and coordinated for assigned cases timely to meet standards.
- Prepares reports for the Director and/or Administrative staff regarding assigned caseload.
- Performs data entry of client follow up and assigned caseload.
