Title: Community Health Worker
Supervisor: Case Manager
Overview:
The Community Health Worker provides ongoing home visits, assessments, referrals to resources, education, and support to women at risk of a poor birth outcome.
MomsFirst is a program for pregnant women. The objective of the program is to reduce infant mortality and improve low birth weight of newborns. Friendly Inn MomsFirst serves moms who are residents of Central, Kinsman, and Broadway-Slavic Village neighborhoods of Cleveland, Ohio.
Responsibilities:
- Conduct community-based outreach and participant recruitment to pregnant women in need of perinatal and infant health care and supportive services.
- Provide case management for 30 or more participants by conducting home visits twice per month minimum. Case management includes activities such as intake, depression, and developmental screenings, providing referrals and following up, resource distribution, documenting the service provided in an electronic data system and a corresponding paper record file.
- Deliver education on topics, such as breastfeeding, family planning, safe sleep, birth spacing and others using evidence-based curricula.
- Administer questionnaires and screenings to program participants at pre-determined intervals and as needed/indicated. The questionnaires and screens may include: Parent/Child Care Plan questionnaire, Edinburgh Perinatal Depression Scale, Ages and Stages Questionnaire, parent/child background screens, and other screenings.
- Assist in securing participant involvement in program events, meetings, and activities.
- Maintain accurate and up-to-date documentation on all participant activity within 24 hours of contact.
- Participate in and complete all required training, staff development activities, continuing education programs and an annual Saturday event.
- Adhere to MomsFirst Core Value expectations for integrity, communication, empathy, and support.
Required qualifications:
- High school diploma or general equivalency diploma (GED)
- Community Health Worker Certification (required and may be acquired within one year of hire)
- Knowledge of community health and social service resources
- Three plus years of experience in outreach and/or case management
- Proficient in Microsoft Word, Internet navigation and use of electronic data systems
- Strong written, verbal, time management and organizational skills
- Valid driver’s license, active full coverage insurance and dependable transportation
Preferred qualifications:
- Associate or bachelor’s degree in social work or related field
- Community Health Worker Certification
- Knowledge/experience providing trauma informed care
- Bilingual, Spanish preferred

- Ability to work in a variety of settings with culturally diverse families and communities with the ability to be culturally sensitive and appropriate.
- Must have a car and a valid driver’s license, be comfortable conducting home visits and provide own transportation.
- May require working some evenings and weekends.
Typical start and end time: 9:00 am - 6 pm
