Director of Outreach and Prevention
REGULAR HOURS :
Tue-Thurs : 9am-3 : 30pm
Fri : 9am-3 : 30pm (subject to change depending on event)
Sat : 9 : 30am-5pm (subject to change depending on event)

SUMMARY :
The Mobile Unit Primary Care and HIV Care and Prevention Nurse Practitioner provides comprehensive sexual wellness and primary healthcare services to patients, with a special focus on the prevention, diagnosis, and management of HIV on the Friends Mobile Unit. This role involves delivering primary care, managing chronic illnesses, conducting routine health assessments, and implementing prevention strategies. The Nurse Practitioner will collaborate with healthcare providers, support staff, and community organizations to ensure high-quality, patient-centered care.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
- Provide comprehensive primary care services, including diagnosis, treatment, and management of acute and chronic illnesses.
- Conduct routine health assessments, screenings, and immunizations.
- Develop and implement individualized care plans for patients, focusing on preventive care and health maintenance.
- Prescribe medications and therapies as appropriate, following evidence-based guidelines.
- Manage the care of patients living with HIV, including prescribing and monitoring antiretroviral therapy (ART).
- Provide pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) services to at-risk individuals.
- Educate patients about HIV prevention strategies, risk reduction, and healthy lifestyle choices.
- Provide counseling and support to help patients manage chronic conditions and adhere to treatment plans.
- Work collaboratively with physicians, nurses, and other healthcare providers to deliver coordinated, high-quality care.
- Participate in multidisciplinary team meetings to discuss patient care plans and progress.
- Establish and maintain relationships with community organizations and public health agencies to enhance service delivery.
- Maintain accurate and complete medical records, documenting patient assessments, treatments, and follow-up care.
- Ensure compliance with all federal, state, and local regulations, as well as organizational policies and procedures.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Provide support for pre-authorizations for medical procedures and medications.
- Complete proper and timely documentation of services in writing and in Athena, CAREware, Evaluation Web, Redcap, Apricot, and any other database, as needed.
