MDS Coordinator Job Description
Purpose of the Position:
To oversee and facilitate the completion of resident MDS assessments at Friendship Haven. This position will follow all SNF/ICF residents in all neighborhoods and make sure they are getting the correct plan of care. Throughout the assessment and while planning care, ensure compliance with Federal and State regulations.
Essential Duties:
·Accountable for tracking and completion of the MDS process for SNF/ICF residents from beginning to submission. This includes resident/staff interviews, all MDS sections, development of CAAs, associated paperwork with the MDS and development of the care plan including the baseline care plan.
·Will be responsible for, and coordination of, all SHC admissions and referrals. Including managing the referral phone.
·Assist in leading care plan conferences for all residents.
·Attend and lead huddles and/or meetings with collaboration from other neighborhood leaders and the therapy department.
·Assists the Coordinators with residents/family/staff concerns with the residents, as needed.
·Assists neighborhoods in day-to-day nursing staff vacancies, as needed and able.
·Work with therapy daily to ensure the best care for SNF/ICF residents.
·Collaborate with the therapy team and provide correct ICD-10 codes for paperwork and billing.
·Collaborate with billing department regarding residents, as needed.
·Complete and track all skilled certification forms for all skilled residents.
·Assist with PDPM audit, as needed.
·On call rotation with SHC Supervisor Nursing team, as assigned.
·Assist Coordinator with discharge planning and implementation for residents, as needed.
·Must have knowledge of CPR and IPOST.
·Serves on Friendship Haven Committees, as needed.
·Other duties as assigned.
Minimum Requirements
·Education: Licensed RN, a background in Human Services is beneficial but not required.
·Must be CPR certified.
·Willing to become a Notary and keep credentials current.
·Long Term Care experience preferred, but not required.
·Rehab Certification is beneficial, but not required.
·MDS certified or the ability to become certified.
·Knowledge related to PDPM is beneficial but not required.
