Addresses in partnership and collaboration with other health care professionals the patient's wellness, acute illness, chronic disease, disability, and end -of-life needs. Responsible for patient advocacy, coordination of nursing and other health services, implementation of nursing services and continuity of care. Provides patient education when needed.JOB REQUIREMENTS / PREFERRENCES : Graduate of an accredited school of nursing required, and current unrestricted Wisconsin RN license required. None required; at least one year medical / surgical nursing preferred. CPR certification required.
