POSITION PURPOSE |
- Shadows and works to assess patient's medical condition, psychosocial concerns, home environment and additional information to provide appropriate discharge placement. Observes and learns communication styles with family and/or significant other, staff and outside agencies to discuss discharge-planning alternatives. Assists patients/families in coping skills and provides emotional support. Learns consultation reports and documents activity in patient medical record. Performs other related duties as needed.

- Progressing in an accredited social work program.

MINIMUM EXPERIENCE REQUIRED |
- Introduction to social work class is in progress.

- Experience in the health care arena preferred.

PRINCIPLE ACCOUNTABILITIES AND ESSENTIAL DUTIES |
- Developing skills to complete a patient’s medical and psychosocial assessment for discharge planning.
- Learning proper communication skills with multidisciplinary team.
- Documents assessments in the patient’s medical record under supervision.
- Assists patients/families in coping skills and provides emotional support
- Developing knowledge of and ability to discuss financial alternatives with patients and families.
- Observes and participates in interdisciplinary meetings.
