Job Summary:
As part of the Care Coordination Team, the RN Care Coordinator is responsible for Chronic Care Management of high risk and complex care patients. Emphasis on building relationships with provider, patient and family/caregiver to increase self-management of complex disease states including heart failure/heart disease, asthma, COPD, diabetes and supporting workflows to close gaps in patients' chronic and preventative health care needs. Collaboration with PCP and Care Teams to establish patient-centered goals and a plan of care to support achievement of those goals, closing gaps in care and to meet patient needs during vulnerable times of transition across care continuum or adjustment to new diagnoses. Other duties as assigned.
EXPERIENCE DESCRIPTION:
A minimum of 4 years of experience as a Registered Nurse is required. Experience in Medical Surgical or primary care nursing is preferred. Experience in critical care, home health or cardiac/pulmonary nursing is preferred. Previous experience as a case manager is preferred.
EDUCATION DESCRIPTION:Professional knowledge of nursing theory and practice at a level normally acquired through completion of a minimum of two years education at an accredited School of Nursing in order to be eligible for licensure as a Registered Nurse is required. Bachelor's degree in Nursing is preferred.
SPECIAL SKILLS DESCRIPTION:
Highly organized with the ability to manage time and prioritize work flow. Ability to be self-directed and work autonomously. Demonstrate effective verbal and written communication skills. A high degree of positive energy and the ability to rapidly initiate positive relationships with others to develop trust. Demonstrates creative solution-based problem solving, critical thinking skills, and clinical judgment. Willingness to accept and adapt to change. Possess effective conflict management and motivational interviewing skills. Maintaining knowledge and awareness of organizational standards for chronic and preventative care management Keyboard skills and experience working with EMR and Microsoft Office required.
LICENSURE DESCRIPTION:Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required upon hire.
