JOB SUMMARY: The Community Connector position works with the Assistant Services Director/DSPA and the Community Connector Event Coordinator to facilitate community inclusion through group/individual social activities. This includes collaborating with the Community Connector Event Coordinator to identify desired activities, transport to events/activities, provide social coaching specific to the individual’s social goals and document billable services. This position is specific to an Assisted Living Home and will work full-time in this location. The Community Connector will be trained in this home and will be back-up the 8am-4pm or the 4pm-12pm home shifts. This position requires time flexibility from 8am until midnight and must be able to adjust workdays if needed to attend events on the weekend and outside of the Kenai/Soldotna area. Responsible for ensuring that policies, procedures, and practices comply with applicable state and federal regulations; and are consistent with the agency’s mission statement, core values, and service principles. This is a non-exempt position.

o Ability to work quickly and efficiently within various software programs.
ESSENTIAL FUNCTIONS:
o Work with clients to facilitate socialization within their community and in Alaska through waiver day habilitation services.
o Have flexibility from 8am until midnight and be able to adjust workdays if needed to attend events on the weekends.
o Implement scheduled activities by the Event Coordinator and transport, participate and supervise educational, recreational, and social activities for individuals that receive services through FCS.
o Will be back-up for the 8am-4pm or the 4pm-12pm home shifts due to unplanned absences.
o Encourage individuals to develop skills and to participate in social, recreational, or other therapeutic activities that enhance community living skills, interpersonal skills or develop social relationships.
o Monitor clients' physical and emotional well-being and report unusual behavior or physical ailments to treatment team.
o Observe and influence clients' behavior, communicating and interacting with them by teaching, counseling, or supporting them.
o Participate with treatment team in the development of treatment goals, behavior support, and crisis/safety plans.
o Write day habilitation notes in Alayacare for services provided.
o Ensure the completion of required documentation promptly including service notes and medication log with team collaboration.
o Monitor health and safety issues, make necessary reports to appropriate person or persons, i.e., performing Critical Incident Reports, etc.
o Assist with personal care needs and medication administration.
o Assist in maintaining client files.
o Collaborate with treatment teams to ensure seamless provision of recreational services.
o Provide transportation to clients using personal car, agency vehicle (if available) or rented passenger van in the community and Alaska.
o Maintain and reconcile receipts.
o Maintain confidentiality of client information in keeping with agency policy and procedures.
o Participate and lead activities that occur in the evening, over the weekend, on holidays and potentially activities that are multiple days/weekends long.
o Participate in Quarterly House Meetings.
o Notify the social media coordinator of activities weekly so they can be posted on the agency social media sites.
ADDITIONAL JOB DUTIES:
o Help FCS meet its quality improvement, evaluation, and reporting.
o Contribute to developing outcomes and indicators and setting targets.
o Apply data to improvement practices and outcomes; and
o Use program and client outcome data to improve service delivery.
o Other related duties as assigned.
