JOB SUMMARY: This non-exempt, full-time position is responsible for assisting the waiver program within FCS, ensuring that policies, procedures, and practices comply with the Council on Accreditation (COA) standards, state, and federal law, and are consistent with the agency’s mission statement, core values, and service principles. This position primarily provides administrative support to the compliance programs in the Human Resources and Operations Department as it relates to Assisted Living Home compliance, specifically to medication assistance and administration as well as home maintenance. Responsible for managing and coordinating all aspects of medical appointments and related services for clients in Assisted Living Homes. This role ensures efficient scheduling, transportation, follow-up, and documentation of medical care for adult clients.

ESSENTIAL FUNCTION/JOB DUTIES:
- Appointment Management
- Schedule and coordinate all medical appointments for clients
- Maintain an up-to-date calendar of client appointments
- Ensure timely communication of appointment details to relevant staff and clients
- Transportation Coordination
- Arrange transportation for clients to and from medical appointments
- Coordinate with drivers or transportation services as needed
- Documentation and Follow-up
- Complete and manage all necessary paperwork related to client appointments
- Ensure proper follow-up on medical recommendations and treatment plans
- Maintain accurate and up-to-date medical records for all clients
- Medication Administration Record (MAR) Management
- Assist in editing and updating client Medication Administration Records as needed
- Ensure accuracy and compliance with medication administration protocols
- Training Support
- Serve as a backup trainer for Safe Transfers and eMAR training for all staff
- Assist in developing and updating training materials as needed
- Annual Appointment Tracking
- Monitor and track all client annual appointments
- Ensure timely scheduling of required annual check-ups and screenings
- Communication and Liaison
- Act as a liaison between clients, healthcare providers, and internal staff
- Communicate effectively with clients, guardians, and healthcare professionals
- Quality Assurance
- Monitor and ensure compliance with all relevant healthcare regulations and standards
- Participate in quality improvement initiatives related to client healthcare
- Emergency Response
- Assist in coordinating medical care during emergencies
- Ensure proper documentation and follow-up for any emergency medical situations
- Maintenance Compliance
- Assist with Maintenance Tickets
- Coordinate with contractors and vendors
- Complete purchase requests to ensure prompt vendor payments
- Delivery orders to Assisted Living Homes
- Administrative and Direct Service
- Attend meetings as required.
- Ability to use common office equipment, including desktop computer, copier/scan/fax machine.
- Answer telephones/emails and text messages, direct calls, and take messages.
- Communicate with employees, program managers, clients, and others to answer questions and disseminate or explain employee and client schedules information.
- Customer service with clients, employees, Community partners and visitors.
- Organizes and maintains client files, forms, and resources.
- Electronic quality assurance and compliance.
- Maintain confidentiality as per policies and practice.
- Prepare, file, and store current and archived material in the database.
- Must be knowledgeable of the overall services provided by Frontier Community Services to be able to assist with client and community questions regarding program information and perform the referral process.
- Assists in setting up special events (e.g. local conferences, in-service training, health fairs, etc) that may occur on the evenings and weekends.
- Provide coverage for client shifts as needed and assigned by the supervisor that may occur on the evenings, weekends, and holidays.

HEALTH AND SAFETY REQUIREMENTS:

- Work is performed in multiple settings both in the office and community; Must have an AK drivers license and have reliable transportation.
- Certified in CPR/First Aid/AED.
QUALIFICATIONS, EDUCATION AND EXPERIENCE REQUIRED
- High school diploma or equivalent
- 2 years of experience in healthcare coordination or related role
- Strong knowledge of medical terminology and basic healthcare processes
- Excellent organizational and time management skills
- Proficient in use of electronic health record systems and Microsoft Office suite
- Strong communication and interpersonal skills
- Preference given to experience working in a human service setting and/or scheduling.
- Typing speed of at least 45 words per minute.
- Valid Alaska Driver’s License and current Auto Insurance
- Ability to transport individuals in your own vehicle.
