Description

The Community Case Management Program Coordinator provides administrative and clinical oversight to direct service staff serving individuals with behavioral health disorders and housing instability.  The Coordinator functions as a champion for evidenced based practices/best practice models, including Housing First, Motivational Interviewing, Trauma Informed Care, Harm Reduction and Integrated Care. Responsibilities include onboarding and supervision of case management staff, monitoring of staff performance and client outcomes, and ensuring compliance with program requirements.  The Coordinator is an energetic, mission-focused leader who drives improvements in the program and coaches staff to achieve high quality outcomes for clients.

Pay: $50,000.00 - $54,000.00 per year

Qualifications


Required: Bachelor’s degree in social work or counseling.  LSW/LPC.  (May consider candidates who are not currently licensed, but will obtain licensure within 6 months of hire.)  A minimum of two years related work experience in a non-profit or health related organization.

Preferred: Master’s degree, LISW/LPCC eligible.

Salary : $50,000 - $54,000
