- Conduct clinical field evaluations of patient referrals
- While under supervision, assess the needs of assigned referrals
- Develop solutions based on needs assessments, that meet identified needs, which result in inpatient and/or outpatient referrals.
- Educate patients/ residents and their families on facility/ hospital services and admission standards.
Qualifications & Requirements
- Ideal candidate must be a skilled communicator, director and motivator; Able to organize and prioritize many tasks effectively
- Must be a licensed RN or LPN/LVN in good standing with the state (as required)
- Previous experience with hospital or long-term care marketing preferred
- Reliable transportation required
