As a key member of our team, the Case Manager will be responsible for providing advocacy and case management services to clients, developing strong relationships with residents, and ensuring their care and safety.

As a key member of our team, the Case Manager will be responsible for providing advocacy and case management services to clients, developing strong relationships with residents, and ensuring their care and safety.
Duties and Responsibilities:
- Provide advocacy and case management services to assigned residents.
- Develop strong relationships with clients and provide support and guidance as needed.
- Complete all clinical documentation and casework, including assessment, individual action plans, group progress notes, weekly progress notes, discharge summary, and transition/discharge/aftercare planning.
- Maintain updated clinical notes and reporting in accordance with established record keeping procedures.
- Facilitate daily evidence-based psychoeducation groups and other recovery-based groups.
- Administer and complete EIM/ESM forms and adhere to funding sources' requirements.
- Directly support contact with residents to continually assess their individual needs, provide information, discuss problems, set limits, and encourage growth and development.
