Position: Case Manager
Location: Longmeadow, Massachusetts
As a Case Manager at Pioneer Behavioral Health's outpatient substance use disorder day treatment program, you'll play a vital role in supporting clients on their journey to recovery. You'll provide crucial support and guidance, including developing individualized aftercare plans, connecting them with community resources, and advocating for their needs. You'll collaborate with a dedicated team of professionals to ensure clients receive the highest quality of care.
Specific Duties:
1. Connect clients to appropriate aftercare settings by:
- Obtaining necessary releases of information (ROI) from clients.
- Providing documentation to outside parties when necessary for client continued programming (e.g., court attorney, FMLA letters, or supporting documentation).
- Staying in close communication with the clinical team to determine aftercare recommendations.
- Contacting potential referring agencies.
- Coordinating with clients, families, and receiving providers for successful transitions.
2. Gather updated client information pertaining to insurance coverage on a weekly basis.
3. Assist with discharge planning and documentation by:
- Following up with the Clinical Director to confirm discharge date and time.
- Completing required discharge planning paperwork.
- Coordinating transportation for clients' transition from the facility to the receiving facility if applicable.
4. Facilitate interventions with clients displaying behavioral or mental instability and clients at risk for leaving Against Medical Advice (AMA) by:
- Contacting appropriate referents and family members for whom the agency has a release of information.
- Enforcing program rules and expectations and reporting infractions to the Therapist and Clinical Director.
5. Complete DAS, RAS, and PHQ-9 assessments along with weekly case management notes on or before required due dates.
6. Monitor client conformance with the program schedule, including group attendance, report problems, and inform supervisor and agency comptroller for billing purposes.
7. Make daily assignment checks and discuss results with each client.
