Position: Lead Case Manager (Full-Time; Hybrid) Contingent upon contract award.
Location: Franklin Street VA Clinic, Community Resource and Referral CenterCenter (CRRC), Washington, DC
We are seeking a highly experienced Lead Case Manager for the HUD-VASH program to assist veterans with obtaining and maintaining stable housing in the community by providing case management so that homelessness is no longer an issue.
The Lead Case Manager must have a minimum of a Bachelor's degree from an accredited institution in Sociology, Psychology, Social Work, Mental Healh Counseling or Family Therapy with at least two (2) years of demonstrated case management with high needs/high intensity of services clients. Due to the complexity, physical and/or mental health of the veterans we will be serving, a Master's level case manager with at least one (1) year of demonstrated case management with high needs/high intensity of service clients is preferred. Assists with the oversight of other Contract case managers on the team.
Other duties as assigned.
LCSW must be active, current and unrestricted
Curren Basic Life Support (BLS) must be submitted with resume to be considered for this position.
Strong organizational, communication and leadhership skills.
Ability to work effectively in a team oriented environment and meet deadlines.
