Job Description: Resident Care Coordinator - CCRC (Continuing Care Retirement Community)

Position Overview:
We are seeking a compassionate, organized, and experienced Resident Care Coordinator to join our team at Gardens Care Senior Living in Arizona. The ideal candidate will work closely with residents, their families, and the care team to ensure the highest level of care, quality of life, and safety for each resident. The Resident Care Coordinator will serve as a liaison between residents, families, medical professionals, and staff to provide exceptional care in a holistic, supportive, and respectful environment.

Key Responsibilities:

Resident Care Management:
- Coordinate and oversee care plans for residents across all levels of care (independent living, assisted living, memory care), as needed.
- Conduct initial assessments, regular evaluations, and updates to care plans in collaboration with residents, families, and medical providers, as needed.
- Ensure care plans are followed, and services are delivered in a timely and efficient manner.
- Monitor residents’ health status and work with nursing staff to identify changes in condition, needs, or care requirements.

Family and Resident Communication:
- Serve as the primary point of contact for families regarding residents' care.
- Regularly update families on resident progress, concerns, and any changes to care needs.
- Assist families in navigating any concerns and mediate solutions in a timely and compassionate manner.

Coordination of Services:
- Work with interdisciplinary teams, including physicians, nurses, therapists, social workers, and other staff, to ensure optimal care coordination.
- Facilitate referrals to outside services or specialists as necessary.
- Ensure that resident care meets all regulatory and facility standards, maintaining compliance with state and federal regulations.

Resident Advocacy:
- Act as an advocate for residents to ensure that their preferences, rights, and dignity are upheld.
- Foster a culture of respect, kindness, and empathy in all interactions.

Medication Oversight and Management:
- Oversee the administration of medications, ensuring compliance with physician orders and regulatory standards.
- Ensure timely and accurate documentation of medication administration and any adverse reactions.
- Work with the nursing team to monitor residents’ responses to medications and collaborate with physicians on any necessary adjustments.
- Maintain accurate and up-to-date medication records for each resident and ensure that medications are stored and handled in compliance with safety protocols.

Documentation and Reporting:
- Maintain accurate and up-to-date care records, including care plans, progress notes, medication logs, and assessments, in accordance with facility policies and regulations.
- Provide timely reports and documentation for facility audits and regulatory reviews.

Staff Support and Training:
- Provide support and guidance to direct care staff (nurses, aides, etc.) regarding care plans, medication management, and resident needs.
- Assist with training and educating staff on care protocols, customer service, regulatory compliance, and medication administration.

Shift Work:
- Work shifts as needed to ensure continuous care and coverage, including weekends and holidays as required by the needs of the residents and facility.
Qualifications:

Education:
- Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field (preferred, but not required).
- LPN (Licensed Practical Nurse) or RN (Registered Nurse) licensure (preferred, but not required).

Certification:
- Certified caregiver in Arizona (required).
- Valid CPR and First Aid certification (preferred).

Experience:
- Minimum of 2-3 years of experience in long-term care, senior care, or related healthcare settings.
- Previous experience in a CCRC, skilled nursing, or assisted living environment is highly desirable.
- Experience working with interdisciplinary care teams is preferred.

- Ability to handle sensitive and confidential information with professionalism.
- Knowledge of regulatory requirements and standards in senior care and CCRC settings.

Work Environment and Physical Requirements:
- This role requires work in a residential community setting and involves regular interaction with residents and their families.
- Occasional physical effort may be required, including moving equipment, assisting with mobility, or lifting up to 25 lbs.
- The position may require occasional weekend, evening, or holiday shifts to ensure 24/7 coverage.
