Title: Crisis and Respite Services Coordinator
Direct Supervisor: BHA Program Director
Full Time: Part time or Full time
Computer Security: Level 7 – Access to paper & electronic records of adults and children actively receiving services
Scope of Job: Oversees the quantity and quality of rehabilitation service to adults using Crisis and Respite Services.
Job Duties:
1. Updates at the beginning of work shift by: a) emptying and reading all correspondence from personal mailbox; b) reading and responding to email; c) listening and responding to voicemail; d) reading Anchor update report; e) reading all required updates including the team bulletin board; f) reviewing critical incidents to get a working knowledge of the current situation and possible interventions needed.
2. Establishes a professional, caring relationship with the people served by the team. Has awareness of and is sensitive to each person's (and of co-workers’) cultural, racial, and ethnic background. Provides regular supervision and feedback to team members regarding the quality of their work. Conducts routine team meetings; guides and supports the team as needed; focuses attention on issues of importance to the team and the agency. Completes routine evaluations for the Crisis staff. Scheduling of team members vacations and holidays. Available via agency cell phone for on-call guidance and emergencies.
3. Monitors the Compass Center, ensuring all service targets and outcomes are met; focuses team attention on service user satisfaction issues. Conducts quality assurance by reviewing all Individual Treatment Plans (ITPs), ensuring that they meet agency and regulatory standards. Completes and enters authorizations for Respite Services and reauthorizations for Crisis Services in a timely manner and follows-up through the appeal process when warranted. Provides guidance and oversight to HSWs on quality assurance and improvement planning on the Individual Treatment Plans (ITPs) of the people receiving services; ensuring planning and documentation are in accordance with regulatory standards (including obtaining of required Health Status Reports) and meet the needs of the individual being served. Follows-up with Critical Incident Reports when written.
4. Oversees and maintains the quality and accuracy of the team bulletin board and all other communication throughout the team. Updates at the beginning of each shift in accordance with agency policies.
5. Oversees all Human Service Workers work in the area of educating Compass Center service users on obtaining needed resources and benefits in the short amount of time being supported by the team. Provides technical assistance to staff to ensure all available benefits are accessed; provides liaison with agencies providing the benefits; tracks and evaluates entitlement status while person is using the Compass Center service. Monitors the uninsured status of people using the Compass Center service.
6. Conducts quality assurance by reviewing all records of the person being served, ensuring that they meet agency and regulatory standards. Provides guidance and oversight to HSWs on quality assurance and improvement planning.
7. Completes management reports including such things as indicating the status of certain designated critical indicators including utilization of services and other areas as requested by the BHA Program Director. Reviews daily Case Notes. Reviews and corrects monthly check of team billing.
8. Oversees team budget. Provides feedback to the BHA Program Director concerning team service targets and consumer satisfaction.
9. Participates as a member of the leadership team. Attends meetings as required, including monthly leadership meetings, quarterly staff recognition events and full staff meetings. Focuses on meeting all COMAR regulations, CARF standards, and HIPAA laws and encourages team members to do the same. Attends certification/recertification training as scheduled. Completes all other training as scheduled/assigned.
10. Performs other tasks related to the overall scope of the job as assigned.
