What are the responsibilities and job description for the Human Service Worker - Residential position at Garrett County Lighthouse, Inc.?
Scope:
Provides generalized staff coverage and specific interventions to people living in the Residentail Rehabiliatation Program in the community.
Job Duties:
1. Updates at the beginning of work shift by: a) emptying and reading all correspondence from personal mailbox; b) reading and responding to email and voice mail; c) reading all required updates including the team bulletin board; d) reviewing Special Plan information to get a working knowledge of the current situation on the team and interventions needed; e) obtaining and covering an agency cell phone if assigned and responding to calls/texts. Checking messages when necessary.
2. Establishes a professional, caring relationship with all people being served by the team. Has awareness of and is sensitive to each person's (and of co-workers’) cultural, racial, and ethnic background. Facilitates voluntary participation in activities and events related to the person's background.
3. Provides on-site and off-site interventions as outlined in each person’s Individual Rehabilitation Plan (IRP) as directed by the HSW3. These supports to residents may include, but are not limited to, medication monitoring; support to appointments; assisting with activities of daily living, encouraging socialization opportunities, etc. Provides additional support to people as directed by the HSW3, including meal planning, shopping, including carrying groceries and putting groceries away after purchase, food storage, meal preparation and clean-up. Also assists with residents moving in or out of the home, including packing/unpacking and assisting with luggage, if needed.
4. Assists the HSW3 with assessing each person’s progress toward accomplishing rehab goals as part of the monthly progress note review. Teaches skills.
5. Helps maintain the home by supporting people to clean and maintain their homes, or if the person doesn’t help, performs the cleaning directly, including taking out the trash. Keeps the office area clean and organized. Assists with outside maintenance, including snow removal, gardening, sweeping, etc.
6. Encourages each individual to work toward their recovery. Offers support, hope, encouragement, compassion and empathy to individuals who are struggling with their recovery.
7. Documents interventions via the Electronic Health Record (EHR). Uploads proper documentation according to policy, when asked. Reports critical situations via the designated electronic system.
8. Completes all requirements at the end of the shift including: a) updating of bulletin board and documentation of shift notes; b) final checking of and responding to voicemail; c) returning and charging of cell phone; d) verbally updating next shift; e) when driving an agency vehicle, ensures its cleanliness and refuels as necessary; f) completion of timesheets and mileage sheets, when due.
9. Attends meetings as required. Trains new staff and interns the job duties of the team’s HSW staff when an HSW3 is not available. Focuses on meeting all COMAR, CARF, HIPAA and other regulatory requirements. Attends certification/recertification training as scheduled. Completes all other training as scheduled/assigned. Understands role as an essential employee.
10. Performs other tasks related to the overall scope of the job as assigned.

Qualifications for this Position:
1. High School Diploma or GED equivalent. * Bachelor’s, or Master’s degree allows for annual education stipend.
2. Strong interpersonal skills.
3. Good documentation skills.
