What are the responsibilities and job description for the Social Worker II - Visitation Center Case Manager position at Gaston County Government?
Gaston Social Services: Protecting, caring for and empowering children and adults who are temporarily or permanently unable to care for themselves through the administrating of federal, state and county programs focused on improving the well-being of our citizens.
THIS IS A GRANT FUNDED POSITION
The duties listed below are not all of the duties that may be assigned but are those that are considered as essential for an employee to perform.
- Coordinate and synchronize support services provided by multiple agencies for families in crisis using a strength-based child and family team approach. Collaborate with child-serving agencies (health, mental health, education, etc.) in assisting Children and Family Services for children in care, focusing on individual assessments of each child's needs.
- Conduct intake assessments on families to identify the needs and goals of the family. In coordination of the assigned social worker and court orders, an individualized plan will be developed with the family, in an effort to provide steps needed to reunify the family.
- Provide supervised visitation to families in a safe and supportive environment. Provide documentation and feedback to the assigned social worker regarding visitation.
- Conduct regular follow up meetings to monitor progress of case plan and participation in achieving reunification goals. Monthly assessment of court orders to evaluate effectiveness of current services and identify ongoing concerns or needed referrals. Provide trauma resources for providers, survivors, and their families, maintaining a continuity of care across service systems. Participate in Children and Family Team meetings, Permanency Planning Reviews, and court dates to support families.
- Monitor families by tracking all direct service data, including type of referral/service, duration of participation, and outcomes throughout the duration of the case. Document all progress notes within 48 hours post-session and submit to the coordinator for review. Complete monthly reports on outcomes, providing a summary for the coordinator and quality assurance.
- Enhance community collaboration by identifying resources and establishing meaningful relationships with community partners to support families in achieving their goals. Attend and present cases at the Visitation Center Advisory Board on a monthly basis.
- Perform other program-specific responsibilities as required.
A valid driver's license is required.
