Role Description
This is a hybrid role located in Portland, ME, with the option for some remote work, as a Case Manager at Gateway Community Services LLC. The Case Manager will be responsible for providing, coordinating care services, advocating for clients, conducting assessments, and developing individualized care plans.
Qualifications
- Case Management Experience; MHRTc required
- Assessment and Care Planning abilities
- Strong interpersonal and communication skills
- Knowledge of Mainecare and social services
- Ability to work independently and collaboratively
- Experience working with refugees and immigrants is a plus
- Bachelor's degree in Social Work, Psychology, or related field
