Summary
As an Adult Case Manager at Hope For All Community Services LLC, you will play a vital role in supporting individuals facing behavioral health challenges. This position is essential in helping clients navigate their treatment plans and access necessary resources to improve their quality of life.
Compensation: $33.00 per hour billable hours; $15.00 per hour administrative time.
Responsibilities:
- Develop and implement individualized care plans for clients based on their unique needs.
- Conduct assessments to identify clients' strengths and areas for improvement.
- Provide crisis intervention and support during critical situations.
- Facilitate access to community resources, including housing assistance and healthcare services.
- Utilize motivational interviewing techniques to encourage client engagement and progress.
- Collaborate with multidisciplinary teams to ensure comprehensive care delivery.
- Monitor client progress and adjust care plans as necessary to meet evolving needs.
Expectations:
- Responsible for working with up to between 10-15 cases and completing timely documentation (knowledgeable about EHR software plus)
- Complete timely 90 days CSR for each client through Attrezzo/Kepro.
- Participate in weekly group supervision/one-on-one supervision, bi-weekly admin supervision.
Requirements:
- Active MHRT/C certification
- Bachelor's degree in social work, psychology, or a related field preferred.
- Experience in behavioral health or social services is highly desirable.
- Strong understanding of crisis intervention strategies and motivational interviewing techniques.
- Excellent communication skills, both verbal and written, for effective client interaction.
- Ability to work collaboratively within a team-oriented environment.
- Must have reliable transportation.
