Location: Portland/Biddeford base, with travel up to 30 minutes away
Required Certificates and Licenses
· Valid driver's license and in good standing
· Bachelor Degree in Social work or related field (We can also consider candidates whose Bachelor Degree is in an unrelated field if the candidate has at least one year of working experience with children)
· Have attained and maintain MHRT-C certification or be able to attain a MHRT-C during your 90-day probationary period
What is the Behavioral Health Home?
Licensed by the Department of Health and Human Services, Gateway Community Services Behavioral Health Home (BHHO) program serves eligible youth and adults. BHHO services is a team based and needs driven mental health program. Participation in the program provides consumers with support from a team of professionals including a health home coordinator, nurse care manager, peer support worker and clinical team leader. The BHHO team also has access to a licensed clinical social worker, a psychiatrist and a medical doctor for consultation. The team will work with consumers on a monthly basis, to help support them in meeting the goals their identified in their treatment plan. Through a variety of services from assessment through advocacy and evaluation, we seek to:
· Encourage and build independence
· Enhance functioning in home, school and community
· Empower children and adults to achieve their optimal potential
How does it work?
Care Coordinators will first conduct an initial assessment to determine the individual’s medical necessity and eligibility. The assessment reviews the medical, mental health, educational, social and recreational needs. Next, we will develop an individualized treatment plan focusing specifically on the goals and needs identified by the child or adult we are serving. With a better understanding of the individual’s strengths and needs, the case manager can then help find services and support, make referrals, assist with team meetings and education needs, and generally monitor progress to ensure the effectiveness of the plan in meeting goals. Right from the start, the individual or family is involved and up-to-date in the progress made every step of the way.

· Assist with navigating immigration needs
· Assist with scheduling and coordinating appointments
· Secure transportation needs through Modivcare for routine appointments
· Support with finding recreational or social activities
· Collaborate with providers to develop crisis and wellness plans
· Support in managing challenging mental health symptoms
