POSITION DESCRIPTION

The Academy Program Coordinator (APC) provides person-centered services and supports to people with intellectual and developmental disabilities participating in Gateway’s Academy day program. The APC provides leadership and program oversight to ensure that services are person-centered, meaningful, compliant, and safe. The APC is responsible for supervising an assigned group of Academy Instructors. APCs play a vital role in helping people live a meaningful and purposeful life in the direction of their choice. The ultimate role of the APC is to actively teach, coach, mentor, and encourage people to embrace and increase their independence and meet their personal goals.

DUTIES & RESPONSIBILITIES

PERSON-CENTERED SERVICES AND SUPPORTS

  • Follow the PCISP and BSP
  • Actively engage with people throughout the day
  • Provide daily support to people to make progress on their PCISP outcomes
  • Maintain records and documentation per established procedures
  • Provide person-specific training to Academy Instructors per established guidelines
  • Complete and document monthly case reviews
  • Actively participate in curriculum development and oversight for the Gateway Academy day program

SELF-DETERMINATION AND ADVOCACY

  • Work with persons supported to plan and prepare group activities
  • Provide daily support to people to understand and exercise their rights
  • Provide daily support to people to make choices for themselves
  • Provide daily support to people to voice opinions and concerns

COMMUNITY EQUITY

  • Provide support to people to explore community interests, activities, and resources
  • Provide daily support to people to develop friendships, relationships, and natural supports
  • Provide transportation to community activities

HEALTH AND SAFETY

  • Provide support as needed for personal care and hygiene
  • Report incidents timely and accurately per established procedures
  • Implement a person’s High Risk Plans
  • Monitor and document health-related issues
  • Follow all established vehicle safety procedures while providing transportation

FLEXIBILITY AND COLLABORATION

  • Manage the staffing schedule for assigned Academy programs
  • Provide direct supervision of Academy Instructors including monthly performance observations, performance evaluations, and progressive discipline
  • Use flexible and creative approaches to keep people engaged throughout the day
  • Communicate issues timely with Gateway supervisors
  • Attend Individual Support Team meetings
  • Maintain professional interactions with internal and external partners

CONTINUOUS AGENCY IMPROVEMENT

  • Participate in quality improvement activities

SUPERVISION

  • Monitor staffing schedules and communicate hiring needs with Gateway supervisors
  • Verify daily that staff members have accurately documented participant activities
  • Complete Letters of Counseling timely for disciplinary actions
  • Immediately communicate issues requiring investigation to programming leadership and Human Resources
  • Other duties as assigned

MINIMUM QUALIFICATIONS

  • Willingness to work a diverse schedule based on the needs and preferences of the people we support
  • Ability to bend, stoop, sit, stand, reach, and lift items weighing at least 75 pounds
  • Meets 460 IAC 6 minimum qualifications for a direct care staff:

460 IAC 6-14-5 Requirements for direct care staff

Sec. 5. All direct care staff working with individuals shall meet the following requirements:

(1) Be at least eighteen (18) years of age.

(2) Demonstrate the ability to communicate adequately in order to:

(A) complete required forms and reports of visits; and

(B) follow oral or written instructions.

(3) Demonstrate the ability to provide services according to the individual's ISP.

(4) Demonstrate willingness to accept supervision.

(5) Demonstrate an interest in and empathy for individuals.

  • If providing transportation to service participants, meet 460 IAC 6 requirements:

460 IAC 6-34-1 Valid driver's license required

Sec. 1. A provider of transportation services shall ensure that an individual is transported only by a person who has a valid Indiana:

(1) operator's license;

(2) chauffeur's license;

(3) public passenger chauffeur's license; or

(4) commercial driver's license;

issued to the person by the Indiana bureau of motor vehicles to drive the type of motor vehicle for which the license was issued.

460 IAC 6-34-2 Vehicle requirements

Sec. 2. A provider of transportation services shall ensure that an individual is transported only in a vehicle:

(1) maintained in good repair;

(2) properly registered with the Indiana bureau of motor vehicles; and

(3) insured as required under Indiana law.

  • Current negative TB (Tuberculin test) as required by 460 IAC 6
  • Acceptable background checks as required by 460 IAC 6 and Indiana DDRS policy
  • Willingness to use a personal smart phone capable of texting, email, and apps for the purpose of completing service documentation through the AccelTrax electronic system
