Gateway Residential Services has an opening for a Program Coordinator position for the Evansville and surrounding areas. Must have a valid driver's license, reliable vehicle and current auto insurance meeting Indiana limited liability.

  • POSITION DESCRIPTION

Program Coordinators (PCs) provide person-centered services and supports to people with intellectual and developmental disabilities living in their own homes in the community. PCs play a vital role in helping people live a meaningful and purposeful life in the direction of their choice. PCs actively teach, coach, mentor, and encourage people to embrace and increase their independence and meet their personal goals. PCs are responsible for oversight of daily operations in a Supported Living site and for implementing procedures to ensure the quality and integrity of services provided. A PC’s salary is based on a minimum of 32 billable hours per workweek.

DUTIES & RESPONSIBILITIES

Ensure a person’s home and services comply with all rules and regulations as indicated in 460 IAC 6, the individual’s PCISP, and all Gateway policies and procedures.

Supervise Direct Support Professionals. Complete Performance Observation Logs quarterly. Assist the Program Director with disciplinary procedures and performance appraisals. Train Direct Support Professionals on individual-specific needs, risk protocols, and other individualized procedures.

Manage the staffing schedule for assigned home. This includes ensuring that the home is fully staffed 7 days a week, filling vacancies due to call-offs, managing schedule changes, and maintaining scheduling documentation.

Prepare and submit all required paperwork within established guidelines: Program Coordinator Monthly Report, monthly documentation remittance, individual budgets, etc.

Ensure incident reporting and follow-up is completed in compliance with Gateway’s Incident Reporting policy and procedure.

Meet with people to complete meal and menu planning. Coordinate grocery shopping within an established budget.

Ensure that personal necessities are always present in the home. Coordinate shopping for personal necessities within an established budget.

Ensure that the home environment is safe and secure. Complete the Environmental Safety Check Record monthly and document all actions taken to resolve issues. Communicate safety concerns and issues promptly with the Program Director. Coordinate and ensure appropriate completion of monthly emergency drills. Ensure maintenance requests are submitted and repairs are completed as needed.

Ensure that all staff follows financial management guidelines, including submitting and maintaining receipts for all purchases monthly.

Maintain appropriate levels of proper medication, medical supplies, adaptive equipment, and medical treatments in the home. Check in medications monthly.

Assist people with planning Meaningful Day Calendars and requesting appropriate funds.

Provide and/or coordinate transportation to activities, health appointments, etc.

Ensure all medical appointments are attended and necessary follow-up (i.e., changes in medication, diet, etc.) is implemented.

Demonstrate/maintain professional internal and external relationships, cooperation, and partnership with all Gateway employees, managers, and the people we support.

Ensure completion of vehicle safety checks within established guidelines.

Provide on-call support after regular business hours.

Communicate issues promptly with the Program Director.

MINIMUM QUALIFICATIONS

At least 1 year of direct experience working with people with intellectual and developmental disabilities with some supervisory experience preferred

Ability to apply sound, mature decisions Provide quality care to people via Direct Support Professional’s responsibilities.

Willingness to work a diverse schedule based on the needs and preferences of the people we support

Obtain and maintain a working cell phone

Ability to bend, stoop, sit, stand, reach, and lift items weighing at least 75 pounds
