A thriving community mental health clinic is seeking an outgoing, energetic case manager who specializes in working with the Forensic/Criminal Justice population in an outpatient setting. We'd like a case manager who is passionate about helping people in the mental health, substance abuse and competency fields to be part of our HIT (High Intensity Treatment Team Team.
HIT is a program that works with individuals who have been diagnosed with serious mental illness (SMI) and may be involved with the criminal justice system. These individuals may have co-occurring substance use and physical health disorders. Their needs are often complex, and their disorders are often under-managed and further complicated by varying degrees of involvement with the criminal justice system. HIT builds on the evidence-based assertive community treatment (ACT) model by making adaptations based on criminal justice issues—in particular, addressing criminogenic risks and needs. In this sense, HIT is an intervention that bridges the behavioral health and criminal justice systems.
Responsibilities and Duties
Developing and implementing processes that enable clients to transition from referral to services in the least number of steps and time; Maintaining an active caseload of referred clients that are seeking services at Gateway To Success; Conducting Behavioral Outreach and follow-up services until the client is engaged with a Service Plan or discharged; Act as the agency liaison with the Justice System, hospitals and other referral sources in an effort to promote access to care.
This position will include assisting clients with intake paperwork, applications for insurance, barriers in transportation or technology, individual sessions, group sessions, and participating in multidisciplinary treatment team meetings.
Qualifications:
Baccalaureate degree in social work, psychology, sociology or closely related field.
Experience in case management and/or community systems navigation for individuals with co-occurring disorders (preferred but not required).
Comfortable with the mentally ill and criminal justice population.
Requires exceptional flexibility, time management, and documentation skills.
Must be a self-starter and self-motivator.
Must possess a valid Colorado driver's license and have access to reliable transportation during work hours.
Desire to make a difference in the lives of people that truly need a reliable advocate to help them obtain the services they desire and need.
Proposed Schedule:
Monday-Friday 9:00am-6:00pm
Job Type: Full-time
Pay: $18.00 - $23.00 per hour
Expected hours: 40 per week
