What are the responsibilities and job description for the Service Coordinator Supervisor position at Gateways Community Services?
Position Summary:
Responsible for direct supervision and management of Service Coordination (SC) staff. Utilize tools/metrics to ensure compliance and quality objectives related to service coordination requirements in he-m 503 and BDS Case Management Functions. Ensure case management tools and processes are supported and executed based on established policies and processes. Seek and provide guidance to staff throughout elevated issues. Ensure SC staff are proactive in planning to develop clinically and fiscally responsible supports to individuals, families, and teams. Maintaining a minimal caseload at times and providing coverage in the absence of the director is required.
Essential Job Functions:
- Manage primary supervision, evaluation, training and guidance, caseload assignment, reaching benchmarks and performance standards.
- Provide initial and ongoing onboarding, training, andsupport to SC staff
- Engage in Strategic Planning and implementation of related processes to meet agency goals and objectives as defined in the Strategic Plan
- • Process check requests, mileage forms, credit card approvals, for consideration of funding on an individual basis.
- Contribute to development of and maintaining healthy team relations and departmental atmosphere
- Maintain a working knowledge of and adhere to all applicable state and federal program regulations and rules
- Responsibility to support team, participate in team meetings and provide resources for internal and community supports as needed
- Oversee and ensure record retention by fulfilling Gateways Community Service’s needs, administrative rules, Medicaid requirements and meet Audit obligations
- Ensure annual service agreements, budgets, and Service Authorizations are scheduled and completed in a timely fashion
- Participate in the oversight and implementation of department tools/systems and its functionality
- Provide opportunity for and implementation of ongoing staff training
- Present advocacy for the needs and wants of the individual
- Report any human right violations or service concerns
- Promote and protect client rights
- Perform other work-related duties as assigned
- Responsible for promoting health & safety within the agency
- Promote & work towards meeting agency goals and objectives as defined in the strategic plan
- Coordinate and participate in internal and community committees/groups as required, assigned and/or as needed
- Lean Practitioner within the organization including growing Lean in Adult SC
- Compliance with all Gateways Community Services policies and procedures
- Complete all necessary required trainings and continue to attend training
- Work collaboratively with other staff/departments and programs as necessary
Qualifications, Education and Experience:
- Bachelors Degree Preferred
- 2-3 years prior supervisory experience
- Preferred knowledge of budgeting process and procedures in order to use existing financial resources effectively
- Knowledge of service system and community resources
- Good communication and organizational skills
- Proficient in Microsoft Windows XP environment including working knowledge of Word, Excel, Outlook along with relational data base knowledge (Microsoft Access and/or Harmony preferred)
- Able to work independently and maintain confidentiality
- Able to work as part of interdepartmental and interagency teams
- Possess reliable transportation, valid drivers’ license, and applicable vehicle insurance for business travel.
Physical Requirements/Work environment
- Occasional standing
- Frequent sitting, minimum of 5 hours per day
- Occasional walking, bending, and twisting of spine
- Light lifting and carrying, less than 30 lbs
- Visual acuity-worker is required to have close visual acuity to perform job functions
Training and Development Required
Complete all necessary required trainings and continue to attend any annual trainings.
