The HIM Assistant is responsible for support and technical duties associated with collecting, sorting, reviewing, securing, and scanning clinical documents and patient information for inclusion in the medical record. Responsible for accurate and timely prepping, scanning, and indexing of documents into the electronic medical record. Monitors scanned documentation, makes corrections, and tracks / reports errors. Maintains paper chart stored either on or offsite and retrieve when needed in a timely manner. Provides support to internal and external customers for management of health information including answering phones, responding to voice and emails.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Retrieve / receive documents from clinical areas daily and prepare for scanning.
- Verify that that each page contains patient name and medical record number or barcode label and remove staples / paperclips.
- Scan prepped documents into the electronic medical record system according to established procedures to ensure best image quality.
- Index document using the document mnemonic to the appropriate encounter number. Validate that the scanned document is viewable to the end-user.
- Review documents scanned outside of HIM at the point of contact to ensure accuracy and image quality. Make necessary corrections or delete and re-scan when necessary. Retain scanned documents as required.
- Maintain paper charts in file room and offsite storage according to hospital policy. Retrieve chart and deliver to requestor for patient care or release of information. Track paper chart by signing in and out in the electronic record locator in Expanse. Return chart as soon as received back in HIM.
- Perform quality review of scanned documents and contents of medical record. Track and report findings.
- Achieve and maintain established quality and productivity.
- Provide administrative support and customer service for department when needed.
Qualifications :
High school diploma or general education degree (GED) and three years related experience or formal training in health information management and / or equivalent combination of education and experience.
