Job Summary
As a Clinical Dietitian, you will deliver evidence-based nutritional therapy while collaborating with a dedicated healthcare team. Your role will involve assessing individual nutritional needs, developing personalized meal plans, and providing both individual and group counseling on dietary practices.
Key Responsibilities
- Conduct comprehensive assessments to determine patient nutritional needs through interviews, medical records, and collaboration with healthcare professionals.
- Create tailored nutrition plans, considering dietary restrictions, food preferences, and health conditions.
- Document all assessments and recommendations clearly in accordance with established practices.
- Educate patients and families on nutrition topics relevant to their backgrounds and health goals.
- Plan and facilitate nutrition classes for various groups as needed.
- Participate in multidisciplinary team meetings and contribute to patient care discussions and goals.
- Support ongoing nutrition care post-discharge through appropriate resources and referrals.
- Engage in continuous improvement activities to enhance the quality and effectiveness of nutrition services.
- Contribute to research and evaluation studies related to nutrition care.
Work Environment
This position is primarily clinical but may also involve flexible work from home options after an initial training period. You will travel between Geisinger Health System locations, including 65 Forward Buckhorn and 65 Forward Shamokin Coal Township.
