Key Responsibilities
- Deliver essential patient care and services as directed by the assigned unit and under the supervision of a registered nurse.
- Execute patient care tasks in alignment with established clinical protocols as per the RN’s guidance.
- Accurately collect and document patient data to assist in assessments and maintain the integrity of medical records, reporting any abnormalities to the RN.
- Identify deviations in patient responses and communicate these findings to the RN.
- Recognize emergency situations and take appropriate actions as necessary.
- Support patient education initiatives as delegated by the RN.
- Build a strong rapport with patients through focused and therapeutic communication.
- Proficient in basic patient care and services.
- Strong verbal and non-verbal communication abilities.
- Capability to identify emergency situations and act accordingly.
- Accurate documentation and reporting skills.
- Expertise in delivering therapeutic and patient-centered communication.
- High School Diploma or equivalent.
- Valid Wisconsin CNA certification enabling work in Federally Certified Home Health Agencies and Hospices.
- Prior experience in customer service or public interaction is essential.
- Possess a current and valid Wisconsin Driver’s License, along with a reliable and insured vehicle.
