We are seeking a Clinical Pharmacist to join our team at Geisinger Enterprise Pharmacy. Our mission is to provide exceptional patient care through innovative, efficient, and high-quality pharmacy services.
The ideal candidate will be responsible for providing quality pharmaceutical care to patients cared for by Geisinger. This includes developing, recommending, and monitoring evidence-based pharmacotherapy care plans that emphasize patient outcomes and quality of life.
Key responsibilities include:
- Providing safe, efficacious, and cost-effective medication therapy for all patients
- Assessing patients' medications for indication, effectiveness, safety, and adherence issues
- Making rounds with medical services and providing in-services to other healthcare providers when requested and necessary
- Consulting and advising members of the medical staff on matters relating to specific drug therapy problems
- Monitoring and acting on patient laboratory results
- Coordinating activities with other pharmacy personnel to accomplish pharmacy and hospital objectives
Requirements:
- Bachelor's Degree-Pharmacy (Required)
- Doctoral Degree- (Preferred)
