What are the responsibilities and job description for the Liaison - Hospital at Home position at Geisinger?
The Hospital at Home Liaison initiates patient and support system conversations to introduce Hospital at Home and facilitate enrollment into the program. The successful candidate will provide strategic, technical, and operational support to assist medical center facility clinical and non-clinical stakeholders with adopting and implementing the Hospital at Home model of care. Additionally, you'll gather and synthesize data from the local sites to assist the Hospital at Home team with identifying unique and specific opportunities to increase patient enrollment from various clinical areas including the emergency department, inpatient areas, and clinics. You'll also build local site expertise and competency to ensure program sustainability.
At least two (2) years of relevant experience in healthcare, marketing/business and/or patient support is required. This position is full-time, 40 hours weekly and 8 hours per day.
- Initiates patient and support system conversations to introduce Hospital at Home and facilitate enrollment into the program
- Expand the Hospital at Home program by educating health care professionals and patients on the benefits of the programs.
- Responsible for the development, refinement, and delivery of educational materials to clinical teams and patients
- Provide dedicated support for enrolling patients in the Hospital at Home programs
- Gather and provide statistical information in regards of the Hospital at Home program. Including the number of patients participating, number of hospital admissions from patients enrolled in Hospital at Home, cost savings Hospital at Home is providing.
- Build relationships with Case Management and Patient Placement.
- Work with case management and patient flow navigators encouraging them to recommend the hospital at home program.
- Develop criteria for what would make a patient an ideal candidate for hospital at home.
- Educate and support care teams in the identification, screening, and enrollment of patients into hospital at home.
- Identify and drive performance improvement efforts focused on patient identification and enrollment practices with care team.
- Each position will have a primary assignment to multiple hospital platforms within the region.
