What are the responsibilities and job description for the Clinical Hospital Liaison position at LiveWell Centers?
We are currently seeking a Hospital Liaison to add value to our team! We are searching for an experienced Hospital Liaison to assist in building our brand and our facility census.
Job Responsibilities:
The ultimate goal of the External Liaison is to facilitate patient admissions by establishing a strong business presence and relationships with hospitals and physicians within the community.
Act as the point of contact for hospital discharges and facilitate the admissions process for all patients.
Marketing/Educating hospitals, physicians, and other community resources on our philosophy and services.
Creating working relationships with social workers/case managers at the hospital.
Serve as a liaison with health care professionals (i.e. physicians, discharge planners, social workers, health care coordinators, etc.) and agency employees to coordinate safe transitions from medical facilities to the home setting/facility setting.
Screening patient clinical charts, verify payer source and discharge plan
Marketing/social media to develop community outreach and build brand image
Conducts all pre-discharge meetings, interviews, and meet and greets within designated accounts and assigned territories Requirements
Qualifications:
2 years health care marketing experience
Marketing/Business Degree required
Must have experience in the skilled nursing setting
