Job Summary Geisinger Care at Home team is seeking an experienced Nurse Practitioner or Physician Assistant (APP) for a community-based position, covering Lycoming county, Pennsylvania. Geisinger Care at Home APP is a field-based patient direct care position performing health risk assessments, depression screening and addressing care gaps for Geisinger Health Plan Gold members in Lycoming county, Pennsylvania. Office location is Geisinger Health Plan Hughes Center, Danville, PA. Approximately 5-10% of the visits are virtual telemedicine. APP must reside in geographic area due to nature of position. There is no call, weekend, or holiday work. Comprehensive medical / retirement benefits starting the first day of employment. Company vehicle, computer, computer monitor, and cell phone are provided.
