General Description
The Nurse-Family Partnership Referral Process and Procedure Assistant will provide overall administrative and operational support to the Nurse-Family Partnership Program Director, NFP Super visor and the Nurse Home Visitors. The Nurse-Family Partnership (NFP) Referral Process and Procedure Assistant (RP&P) will be responsible for the daily data entry of referrals as described in the Referral Process and Procedure and will report all ongoing updates to the Program Director. The NFP RP&P will assist with scheduling and coordinating CAB meetings, attend all Team meetings, record minutes and be responsible for the distribution of meeting minutes to the CAB and Team in a timely manner.
Essential Duties and Responsibilities
- Data management through the Apricot System and DMCN including collection and coordination of client data.
- Maintain updated records, run reports, and attend trainings on data management. Support nursing staff and program director in data collection and reporting.
- Prepare and/or assist with the variety of quarterly and monthly reports on program activities for NFP or other agency reports and operations.
- Gather statistical data as needed or requested by the NFP Program Director, serve as an assist to the NFP staff and the community regarding program services and methods of operation as determined by program regulation, policy and procedure.
- Coordinate when necessary educational and informative activities that enhance the knowledge of the public and community regarding the program regulations, policies, procedures and services.
- Ensure the adherence to safe work practices and procedures: interpret and apply Federal, State and local health policies, procedures, laws and regulations.
- Prepare and maintain records and logs
- Order, inventory and maintain office supplies
- Manage travel arrangements as required by NFP Program Director and staff
- Establish and maintain cooperative working relationships with those contacted in the course of work both interagency and externally.
- Assist with maintaining and updating Guilford Child Developments' website as it relates to NFP.
- Ensure and maintain confidentiality.
- Serve in supervisory/consultant capacity in the absence of the NFP Program Director to NFP staff related to administrative issues only, all nursing issues will be managed by an experienced Registered Nurse designated by the NFP Program Director.
- Helps client to understand the importance of wellness through early screening procedures, health counseling and education when conducting quarterly home visits with the nurses.
- Supervise nurse home visitors through staffing and quarterly observations of home visits.
- Coordinate parent activities and group socialization with content area specialists and nurse home visitors.
- Conducts periodic family interviews as part of the process to evaluate the effectiveness of the programs interventions.
- Participates in QA/Pl activities;
- Shepherds program expenditures and resources;
- Complete job duties as outlined by Nurse-Family Partnership including conducting team meetings/case conferences, educational updates, reflective supervision, etc.
- Evaluate nurse home visitors annually; develop with nurse home visitors improvement/training plans.
- Provide onsite training through modeling of appropriate techniques.
- Provide program orientation and training for new home visitors.
- Participate in the hiring process of home-based personnel.
- Assist in the identification of training needs for home-based staff in coordination with content area directors and human resources/professional development.
Essential Qualifications
Availability of automobile during working hours and a current, valid North Carolina driver's license is required.
- Participates on agency committees and in community activities as a program representative.
- Must be or become certified in basic CPR.
- Conducts parent training as qualified and when needed. Provides learning experiences for nursing students and other community agencies as appropriate.
Physical, Mental and/or Visual Demands
- Must be able to sit and stand for extended periods of time throughout the day.
- Must be able to lift up to 35 lbs.
- Must be able to bend and stoop.
- Must be able to walk extended periods throughout the day monitoring.
- While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time.
- The noise level in the work environment is usually moderate.
- Specific vision abilities required by the job include close, distance, color, peripheral depth and the ability to adjust focus.
