POSITION - DISCHARGE PLANNER /CASE MANAGER
DEPARTMENT Patient Care Services
POSITION REPORTS TO CLINICAL DIRECTOR OF SOCIAL SERVICES
STATUS Non-Exempt
JOB SUMMARY:
Responsible for the provision of care to meet the physical, psychological, and social needs of all patients. This position ensures that each patient is included in healthcare decision-making, as well as works toward creating and sustaining an environment that humanizes and individualizes the care for each patient. This position serves as an advisor for patients and families in decisions about care for immediate needs, as well as aftercare. Provides range of clinical services to include, but not limited to, daily groups, assessments of individual psychosocial needs, financial limitations, acute treatment, and crisis prevention, as well as posthospital care/discharge planning.
SUPERVISORY RESPONSIBILITY: None
POSITION QUALIFICATIONS:
Combination of experience and education normally represented by a High School Diploma with 1-2 years relevant work experience required. Previous psychiatric experience and education in chemical dependency counseling/clinical methods is preferred.
Knowledge and experience in case management is preferred.
Must be able to form good working and therapeutic relationships.
Must be organized, detail oriented and the ability to maintain confidentiality.
Must possess group work skills.
Must have competent oral, written, and interpersonal communication skills, with patients, families and peers.
Good working knowledge of computers with familiarity of Microsoft Office. Experience working with a team of professional staff desirable.
KEY RESPONSIBILITIES\/JOB ACCOUNTABILITIES:
- Provide short term crisis intervention services.
- Assist patients and families with community resources for post-hospital needs.
- Monitor patient progress toward discharge, ensuring completion of requirements within expected length of stay
- Assist patients and families with securing assistance services, as needed, e.g. Medicaid applications.
- Clear understanding of the PASRR process.
- Act as an advocate for patients as it relates to the courts, treatment centers, and other services as appropriate in order to make referrals and conduct discharge follow-ups.
- Participate in the Probate Process.
- Provide outreach and case management to patients.
- Prepare written reports, group notes and summaries as requested and according to program standards and professional ethics.
- Participate in curriculum development.
- Facilitate daily groups.
- Maintain an efficient, organized work environment.
- Maximize office productivity through proficient use of appropriate software applications.
- Active participation in arranging transportation.
- Ensure patient discharges occur by 10AM each day.

LANGUAGE SKILLS:
English is the primary language of the facility. Ability to read and write, communicate clearly with coworkers and customers and to respond to appropriate questions regarding work process or work product.

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions and decimals as appropriate to the position.
REASONING ABILITY:
Ability to apply common sense understanding to carry out instruction furnished in written, oral and/or diagram form. Ability to define and solve problems and collect data.

Pay: $20.00 - $23.00 per hour
