Job Description:
We are seeking a skilled and compassionate Hospice Registered Nurse to join our team as a Full-Time RN Case Manager. In this critical role, you will provide direct patient care services, assess the needs of patients and families, and collaborate with an interdisciplinary team to develop and implement comprehensive care plans.
Responsibilities:
- Provide high-quality, patient-centered care to hospice patients in their homes, assisted living facilities, or long-term care settings.
- Evaluate patient and family needs, identify areas of concern, and develop individualized care plans that address these needs.
- Collaborate with physicians, social workers, spiritual care specialists, and other healthcare professionals to ensure seamless care delivery.
- Manage assigned caseloads, prioritize tasks, and maintain accurate clinical records.
- Participate in quality improvement initiatives, contribute to performance metrics, and promote a culture of excellence within the organization.
Requirements:
- RN license in good standing; current certification in CPR/BLS
- 1 years of experience in hospice, home health, ICU, geriatric, or oncology settings
- Demonstrated ability to work effectively in a fast-paced, dynamic environment; excellent communication and interpersonal skills
- Flexibility to adapt to changing circumstances, prioritize tasks, and manage time efficiently
