**Job Summary:** We are seeking a highly skilled Nurse Practitioner to join our team as a Flex/Per Diem/PRN Hospice Nurse Practitioner in Hyannis, MA. This role will involve performing Face-to-Face (F2F) encounters with patients, gathering clinical information, and communicating findings to the hospice physician for certification or recertification.

We are looking for an individual with a Master's degree in Nursing, completion of a graduate-level nurse practitioner education program accredited by a national accrediting body, and current state licenses as a registered nurse and advanced practice nurse. National certification as a Nurse Practitioner (ANCC or AANP) and a collaborative agreement in place are also required.

- Makes appropriate recommendations to the IDG based on findings of the F2F assessment.
