Job Description
The RN Case Manager will provide patient-centered care, evaluating patient and family needs and assisting in the preparation and execution of the plan of care. The ideal candidate will possess strong communication and interpersonal skills, as well as a compassionate and empathetic approach to patient care.
Responsibilities
- Providing direct patient care services to hospice patients and their families
- Empowering patients' caregivers through education and support
- Evaluating patient and family needs and assisting in the preparation and execution of the plan of care
- Maintaining all assigned patient clinical records
About You
To be successful in this role, you will need:
- A valid RN license in the state of employment
- At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology
- Strong communication and interpersonal skills
- A compassionate and empathetic approach to patient care
