Key Responsibilities
As a Hospice RN/Care Manager, your key responsibilities will include:
Maintaining primary control and professional management of each patient
Acting as primary liaison between physician, patient/family, and hospice team
Providing direct patient care services to hospice patients and supportive care to their families
Empowering caregivers by educating them on care for their loved ones
Collaborating with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to provide comfort, peace, and dignity
Requirements
To be successful in this role, you must possess a RN license in the state of requested employment (or eligibility to obtain). Additionally, you should have 1 years of nursing experience in hospice, home health, ICU, geriatric, or oncology. A current driver's license and automobile insurance are also required.
