What are the responsibilities and job description for the Hospice Nurse Practitioner position at Georgia Community Hospice?
Job Qualifications:
- Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
- Experience: Two years’ experience as an APN, clinical care setting preferred; palliative, hospice or HH preferred.
- Knowledge & Skills: Nursing skills as defined as generally accepted standards of practice, Comprehensive knowledge of pharmacology, palliative care and end-of-life, Good interpersonal skills, Proof of Hepatitis consent/declination
- Transportation: Reliable transportation, valid and current auto insurance
Essential Functions:
- Travel throughout your work day to multiple locations which may include private residences, assisted living and skilled nursing facilities
- Provides effective communication to patient/family, team members, physicians, and other health care professional
- Assess, plan, implement and evaluate nursing care needs for the patient and their family utilizing the resources of the organization and community
- Collaborate with an interdisciplinary team to promote patient and family choice, assure continuity of care, and enhance the quality of life of dying patients
- Demonstrate critical thinking skills, decisive judgment, and the ability to work under minimal supervision
- Coordinates annual competency sessions to ensure professional staff remains in compliance (eg
- Work under physician direction to complete medical requirements while coordinating a whole person plan of care
- Be accountable for direct and indirect quality care for patients of all ages
- Provide care according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act
- Completes face-to-face encounters with patients, as assigned, and provide clinical assessment results after
