We are seeking a dedicated and skilled Per Diem Nurse Practitioner to join our team and support our commitment to exceptional patient care.
**Position Overview:**
As a Per Diem Nurse Practitioner, you will play a critical role in delivering high-quality medical care to our hospice patients. You will work collaboratively with our interdisciplinary team to assess, plan, and implement care plans that align with the goals and preferences of our patients and their families. Your expertise will be vital in providing comfort and support during this important phase of life.
**Key Responsibilities:**
- Conduct comprehensive assessments of patients’ medical, emotional, and psychosocial needs.
-Interdisciplinary Group (IDG) on-site bi-weekly, Clinical Visits as needs, Perform Face to face as needed
-Chart Audits, Montly QAPI, Staff Inservices( Case Studies)
-Assist with Clinical Supervision as needed, Collaborate with MD
- Develop and implement individualized care plans in collaboration with the hospice team and families.
- Manage and prescribe medications, treatments, and interventions in accordance with the established protocols.
- Provide education and support to patients and their families regarding end-of-life care and options.
- Document patient encounters accurately and timely in the electronic medical record.- Participate in interdisciplinary team meetings and contribute to care plan discussions.- Provide on-call support as needed to address urgent patient concerns.
**Qualifications:**
- Master’s degree in Nursing with a focus on Nurse Practitioner studies.- Current Nurse Practitioner licensure in GA.
- Certification in Hospice and Palliative Care (preferred) or willingness to obtain.
- Minimum of 1-3 years of clinical experience, with prior experience in hospice or palliative care preferred.
- Strong clinical assessment, decision-making, and communication skills.- Compassionate and patient-centered approach to care.
- Ability to work independently and as part of a collaborative team
.-Experience using HomeCare HomeBase highly preferred
