What are the responsibilities and job description for the Hospice Social Worker position at Georgia Community Hospice?
Duties:
-Individual counseling; Group and family counseling; Crisis intervention; Patient advocacy and information and referral.
- Provide social work services to individuals and families in a hospice care setting.
- Conduct assessments and develop treatment plans based on individual needs.
-Assists patient and family in acquiring additional community resources, including Medicaid; serves as liaison with social service agencies as necessary.
- Collaborate with other professionals, such as Nurses, Doctors and medical staff, to ensure coordinated care for clients.
-Completes initial and ongoing psychosocial assessments of the patient’s/family’s response to the terminal illness.
- Stay up-to-date with current research and best practices in the field of social work.
-Participates with Interdisciplinary Team (IDG) in the development and implementation of patient care plans. Serves as member of patient care team, interpreting/identifying psychosocial needs of the patient/family.
Requirements:
- Master's degree in Social Work or related field.
- Valid social work license or certification.
- Knowledge of ICD-10 coding for accurate documentation and billing purposes.
- Strong understanding of psychotherapy techniques and crisis management strategies.
- Excellent communication skills, both verbal and written.
- Ability to work collaboratively as part of a multidisciplinary team.
- Strong organizational skills and ability to manage multiple cases simultaneously.
